Physicals
- You must hand a current physical to your coach on the first day of practice or try-outs for any sport, no exceptions are allowed.
- If the physical is conducted after April 15, it is good for the following school year.
- The MHSAA Physical Form must be used and completed by the student, parents, and the physician. ( Form is at the bottom of the page ). It is best to pick up the form at the Athletic Office.
- Insurance information must be included.
- The bottom portion of the physical card is carried in the team's first aid kit and is used if your student/athlete needs emergency room attention and you are not present.
- The player's fee helps to defray the cost of school insurance and it will cover those costs not covered by your insurance(s).
- In season, interscalastic sports injuries are reported by the parents, and forwarded to the Athletic Department. ( Form is at the bottom of the page )
- An Accident Claim Form must be completed within 20 days of the injury.
- Treatment of injuries must begin within 60 days of the injury.
- Only expenses within 52 weeks of the injury are covered.
- Physical Therapy is limited to $250 per claim.
- Proof of loss must be submitted within 90 days after medical treatment ends.
